While certainly a powerful passive boost (in the region of 500 more buffed health than the Heart of Iron), trinkets with on-use health are rather weak for most bosses, despite the fact that health is a very prominent stat for progression tanks. This is because when a tank stacks stamina, he wants to make his damage income as stable as possible, by removing RNG moments when s/he dodges five times and then gets hit three times in a row and dies. However, an on-use health-trinket is only usable to mitigate the aforementioned burst and only if you know about it beforehand. Activating extra health when you're already down to 23 health isn't going to save you from an extra hit unless it is macroed to other survival cooldowns which most likely would manage on their own.

correction, this item is not like having an extra healthstone. it is simply increasing your maximum health not granting the health as well. but combined with a healthstone it would be more like a passive buff to said healthstone.

Correction's correction. It does. Just like Last Stand and Survival Instincts grant you the health. In such case, those abilities would be near useless and a waste of talent points. Moral of the story, any 'Max health' granting abilities grant the health they entail in certain cases.

Edit: It works as follows. If i gain 30% extra HP for the next 25 seconds, for 25 seconds if my HP is 1 i will gain 30% HP of my NEW total in addition to the higher max. This trinket works the same. Keeps your percentage of health at where it was (Example, if i had 42k health out of 50k health total that would be 84% of my health, say I pop Last stand, my HP would stay at 84% of my NEW total, passively granting me more health. This trinket works just the same, Say i have 50k health max and my current is 44,390 (88.70%) and I used said trinket to gain 4610 health, my new total would be 50k (actually damn close to that, but remains 88.70% health) and max would be 54,610. Does that make more sense? It raises current values and max values to keep you at the same percentage. But Last Stand and Survival instincts will (below 30% health) will always grant the extra percentage of health.

I feel like the cooldown on this trinket should be 1.5 minutes since it only gives a 15s buff.

Most 15s buff trinkets have a 1.5 min CD, while 20s buffs have a 2 min CD.
